nurses of Cherry Hill East
Physicals for the Fall 2009 Sports
season will be given at East on June 2, 2009 and August 11,
2009.

To get a
physical on June 2, be sure to have the signed forms into
Mrs. Walsh (athletics secretary) by May 18 @ 2 p.m
-
June 1 - The
student must come to see the nurse right after school at
2:30 to have ht, wt, blood pressure and vision screened
prior to seeing the physician on June 2.

June 2 -
Physicians will be at East at 2:30 p.m. to perform
physicals.

To get a
physical on Aug. 11, be sure to have the signed forms
into Mrs. Walsh (athletics secretary) by July 27 @ 2 p.m.

August 11,
2009 - Physicians will perform physicals at East from 10
a.m. - 1p.m. @ CHHSE
NOTE: Put your September
grade on these forms. (both the June and Aug forms)

All
medications including over the counter medications require a
physician signature and must be updated yearly.
Many issues arise
that impact your child's learning, participation and ability to
achieve. Please share your child's health issues
(both physical and mental) with the nurses. We are
very creative in assisting you and your child in being
successful and want only the best high school experience for
your child.
