Rules:
1. USA Wrestling Card is Required.
2. Food and drinks must be left outside the room.  
3.  All wrestlers must have wrestling shoes and head gear. 
4.  All behavior must be consistent with that of an appropriate athlete. 

I have read and understand the guidelines and procedures as outlined above in the "Cherry Hill East Open Mat Wrestling Release". I understand the guidelines and procedures and agreed to abide by them. I understand that if I fail to follow these guidelines or violate them in any way, I may not be permitted to remain a participant of the Cherry Hill East Open Mat Wrestling.

Liability/Medical Waiver
My child is in good health and has my permission to participate in the Cherry Hill East Open Mat Wrestling Program. In case of medical emergency, I authorize All Cherry Hill East Open Mat Wrestling personnel to seek medical emergency care for my child. I hereby assume all the risks and hazards incidental to my child's participation in Cherry Hill East Open Mat Wrestling activities, that they may receive while competing or traveling to and from Cherry Hill East and I do hereby waive, release and absolve the Cherry Hill East Open Mat Wrestling directors, instructors, assistants, counselors, volunteers and participants from any claim arising out of injury to my child. For Insurance Purposes - USA Wrestling Medical Information and Waiver Forms must be completed and returned before participation, if you cannot show a valid USA Wrestling Card you may not participate. I agree that the grant and release contained therein bonds the minor and me to all of its terms. Both the wrestler and parent or guardian must sign this statement.
